Effective immediately, a Statement of Health and Delivery Acknowledgment will be a requirement on all individual life policy approvals until further notice. A sample copy of the Statement of Health and Delivery Acknowledgment form as approved in the Interstate Insurance Compact is attached here (forms may vary by state).
This form does not require agent signature, but does require completion and return by each proposed insured (child minors excluded), with signature of both the proposed insured and policyowner before consideration can be given for placing coverage inforce.
If there has been no change in health status since time of application for any proposed insured,
- Upon receipt of signed Statement of Health and Delivery Acknowledgment(s) attesting to such and payment of the initial modal premium, the policy will be placed inforce.
If any proposed insured has had a change in health status since the time of application as indicated in their answers to questions 1, 2 or 3,
- Upon receipt of signed Statement of Health and Delivery Acknowledgment(s) attesting to such, further review will be initiated and communicated by Underwriting and no coverage will be placed inforce.
If Statement of Health and Delivery Acknowledgment(s) is not completed and returned within 60 days of mailing, the policy file will be closed as incomplete and no coverage will be placed inforce.
